Patients will be given a form to complete that will provide the practice with the necessary information to register the patient with the practice.

All new patients will need to provide identification. Below is a list of suitable identification:

• Birth certificate
• Marriage certificate
• Medical card
• Driving licence
• Passport
• Local authority rent card
• Paid utility bills
• Bank/building society cards/statements
• National insurance card
• Payslip
• Letter from benefits agency/benefit book/signing on card
• Papers from the home office
• P45

All new patients will also have to have proof of their address.

Patients are able to specify a preferred practitioner, although this choice cannot be absolute. It will depend on availability and appropriateness. Requests to see the preferred practitioner may result in a longer wait for an appointment.
